Licensed practical nurses (LPNs) are valued members of our health care teams, providing a wide range of care and service. With a focus on promoting wellness and preventing illness, LPNs work to support the diverse needs of our patients and families. LPNs are one of the key collaborators in the Collaborative Care Model.
As a member of the patient's healthcare team, LPNs apply practical nursing theory in the assessment of patients, collaboration in the development of the nursing care plan, implementation of the care plan and ongoing evaluation of the patient. LPNs also provide nursing services independently for patients under direction or guidance of a registered nurse, medical practitioner, or other health care professional.
